Some research suggests that GnRH pulse amplitude and frequency may change with age, with the hypothalamus's signaling becoming less robust. This could explain why older men sometimes respond well to interventions that enhance natural testosterone production (like certain nutrients or weight loss) while others require more direct hormonal support.
Additionally, the clearance rate of testosterone may change, and SHBG binding may increase, meaning that both total and free testosterone bioavailability decline more than might be predicted from simple measurements of circulating levels.

Distinguishing Andropause From Disease
It is critical to recognize that andropause is a normal aging process, not a disease. However, it can coexist with or be accelerated by genuine pathology. Men experiencing andropause symptoms should be evaluated for underlying conditions that accelerate testosterone loss: obesity, metabolic syndrome, type 2 diabetes, sleep apnea, chronic kidney or liver disease, and autoimmune conditions can all impair testosterone production.
Additionally, some medications — corticosteroids, opioids, and others — suppress testosterone production. These should be recognized and managed appropriately in men suffering from andropause-like symptoms.
